A note from Sarah
Dear Diane,
I know reaching out wasn’t easy. Many families start here feeling overwhelmed and uncertain — and most find that having a plan makes a real difference. You don’t need to have all the answers; that’s what I’m here for. Here’s what I’m proposing, and why — take your time with it.
Where things stand
Elaine has been having more forgetful days, and Diane is coordinating from two hours away while her brother visits on weekends. They want to keep Elaine at home as long as it’s safe — and they don’t always agree on what “safe” means.
Now: medication safety and a memory work-up. Soon: home safety refresh. Later: long-term care options, only when the family is ready.
Why a plan matters right now
You've started noticing things
Small changes that add up. A plan turns "I think something's going on" into a set of specific things to check and do.
Something just happened
A fall, a hospital stay, a diagnosis. A plan gets you past the immediate decision and onto what comes next.
You've been doing this a while
Managing it, mostly alone. A plan gets it out of your head and gives you help carrying it.
What this means for you
A personalized care plan
A roadmap for your loved one’s aging journey with what needs your attention now, what can wait, and access to qualified experts when you need them.
A secure family portal
A secure portal you share with your aging advisor and anyone in the family you choose to invite, holding the care plan, key documents, and the decisions made to date.
An experienced voice in the room
Your aging advisor attends meetings with you and any of your providers or specialists — asking what needs asking, making sure you’re represented, and that you leave understanding what was decided.
How we'll work together
A 90-minute consultation to understand your full situation before recommending anything.
I present your Care Plan and get you set up on the portal — next steps and expert resources in one place.
As needed, I am here for you — family meetings, appointments, whatever it takes to keep you moving forward with confidence.
Let's talk about cost
Sarah would be honored to support you and your family.
